Please note: This form is NOT to be used to seek any medical advice - if you do need medical advice, please contact via AskMyGP, either online or by telephoning reception.

 

We aim to respond within 5 working days for administrative requests via email.

 

If you have a more detailed enquiry and require information then please telephone the surgery (01483 202066) using one of the following options :

Option 1: Dispensary- for all medication enquiries, repeat prescription information and advice.

 

Option 2: Reception- for all enquiries regarding appointments, clinics and general information requests.

Option 3: Secretaries- for all admin/ referral enquiries and details regarding any correspondence. You can also reach the Community Gynaecology Service secretary via this option. 

 

Option 4: Test result enquiries.
